Description
Position Description The Case Manager assist clients with a range of concerns including, but not limited to, housing stability needs, financial assistance, location of shelter or housing, job preparedness, etc. Completes in person interviews to address a degree of problems through the completion of comprehensive assessments. Secures information such as medical and social factors that contribute to the client's situation. The Case Managers will often assist clients who have issues including but not limited to neglect, abuse, domestic violence, mental health, homelessness, chemical addiction, trauma, aging out of foster care,child protective services, and parental substance abuse. They will also completed required documentation.
Skills Required Microsoft Suite Entering data in data base including case notes, assessments, e-mail, reports etc. Completing required paper work for Harris County Procedures and HUD Programs In field work; including but not limited to monthly home visits with clients, land lord contact, Housing Authority, social service agencies, check delivery, housing search, meet with clients at encampments, meetings, trainings, attend court, and shelter visits etc. Attending meetings, trainings, communication with direct reports, reading e-mail, reading policy and procedures locating resources in the community, refer clients and link clients to services Disaster related assignments and or other duties assigned
Skills Preferred
Experience Required 1 year experience with vulnerable populations or the homeless
Experience Preferred Preferred: 1 year of working in the Child Protective Services Sector Preferred: 1 year of working with foster children and or families involved in the CPS system
Education Required Bachelors Degree in Social Work
Education Preferred
